Intratimpanic jugulare glomus. Presentation of a case.

Abstract

Introduction: The jugulare glomus is a rare benign tumor, it is a paraganglioma of the head and neck confined to the jugular fossa. Most are benign and slow-growing, but they can invade adjacent structures. The initial symptoms are low density, so delays in diagnosis are frequent. Radiological imaging techniques, especially magnetic resonance imaging (MRI), are the main methods for diagnosis.Material and methods: We present the case of a left jugular glomus.Results: The 63-year-old patient came to our clinic due to intense left pulsatile tinnitus and progressive hearing loss. The otoscopy showed a bluish tympanic membrane, bulging in its lower region that pointed to a pathology of vascular origin. In the audiometry, moderate hearing loss of left transmission was observed. A TAC showed a left jugulare paraganglioma of 2.4 x 1.4 cm with extension towards the middle ear and the cerebellopontine angle. A conservative attitude was decided but the patient began to suffer dizziness for what was once in stereotactic radiotherapy. As a consequence, he suffers from left recurrent palsy with a glottic GAP that does not affect the risk of aspiration. The patient has been proposed to perform a type I thyroplasty that she has rejected at the moment.Conclusion: Although the jugular glomus is a rare entity, it must be considered in the presence of a pulsating unilateral tinnitus. Recurrent paralysis can be a consequence of treatment.
