Utilidad de la biopsia del ganglio centinela en el melanoma de cabeza y cuello. Estudio de cohorte longitudinal retrospectivo

Resumen

Introducción y objetivo: La metástasis linfática oculta ocurre en el 15–20 % de los cuellos clínicamente negativos en el melanoma cutáneo de cabeza y cuello (MCCC). El objetivo de este estudio fue evaluar la utilidad clínica y pronóstica de la biopsia selectiva del ganglio centinela (BGC) en pacientes con MCCC tratados en un hospital de tercer nivel. Método: Cohorte retrospectiva de 273 pacientes con MCCC (2014–2024). Se identificaron los casos con indicación de BGC y se clasificaron según su resultado. Se analizaron variables demográficas, clínicas, histopatológicas y recidiva. El análisis estadístico se realizó con IBM SPSS Statistics v.30, considerando significativo p < 0,05. Resultados: La mediana del índice de Breslow fue mayor en el grupo con BGC positiva (3 mm; RIQ 2,25–4,00) que en el negativo (2 mm; RIQ 1,50–3,38), sin significación estadística (p = 0,169). Se observó recidiva en 4/11 pacientes con BGC positiva (36,4 %) y en 3/22 con BGC negativa (13,6 %) (p = 0,186). Entre los pacientes con BGC positiva, 6 fueron sometidos a linfadenectomía cervical (LC), confirmándose metástasis en 3 de ellos (50 %). La mediana de seguimiento fue de 118,9 meses, sin diferencias entre grupos. Conclusiones: La BGC permitió detectar metástasis ocultas en un porcentaje relevante de pacientes con MCCC. Sin embargo, la limitada potencia estadística impide establecer su impacto pronóstico. Se requieren estudios prospectivos y multicéntricos para definir con mayor precisión su utilidad.
