PET/TC en cáncer de cabeza y cuello: estadificación, tratamiento y detección de tumores sincrónicos

Resumen

Introducción y objetivo: Los tumores de cabeza y cuello representan el sexto tipo de cáncer más frecuente y suelen diagnosticarse en estadios avanzados, lo que condiciona la supervivencia. La tomografía por emisión de positrones combinada con tomografía computarizada (18F-FDG PET/TC) es una herramienta eficaz para detectar enfermedad subclínica, metástasis ocultas y tumores sincrónicos, especialmente en pacientes con alto riesgo o antecedentes oncológicos. Su integración con otras modalidades diagnósticas mejora la precisión en la estadificación TNM, optimiza la planificación terapéutica y reduce procedimientos invasivos. El objetivo de este estudio fue evaluar la utilidad del PET/TC en la estadificación inicial, el seguimiento y la detección de tumores sincrónicos en pacientes con cáncer de cabeza y cuello.

Método: Estudio retrospectivo de 100 pacientes con cáncer de cabeza y cuello sometidos a PET/TC como parte de la evaluación inicial. Se analizaron variables clínicas, histológicas y metabólicas incluyendo el valor de captación tumoral (SUV tumoral) y el valor de captación en ganglios linfáticos (SUV nodal), así como los cambios en la estadificación y el tratamiento derivados de los hallazgos metabólicos.

Resultados: Predominó el sexo masculino (90 %) y el carcinoma epidermoide (84 %). Los síntomas más frecuentes fueron disfonía, odinofagia y adenopatía, con localización supraglótica y orofaríngea. El PET/TC modificó el estadio TNM en el 42 % y el plan terapéutico en el 15 % de los pacientes, detectando además metástasis no identificadas clínicamente y nueve tumores sincrónicos. Se observó concordancia con la anatomía patológica en 57 casos, destacando su utilidad en la planificación del tratamiento.

Discusión: La 18F-FDG PET/TC mostró alta sensibilidad en la detección de tumores de cabeza y cuello, aunque limitada en lesiones pequeñas o submucosas. Su integración con TC y RM mejora la precisión diagnóstica y la planificación terapéutica. Los valores elevados de captación tumoral (SUV tumoral) y ganglionar (SUV nodal) observados podrían asociarse con peor pronóstico. La principal limitación del estudio fue su diseño retrospectivo sin grupo control.

Conclusiones: La 18F-FDG PET/TC es una herramienta valiosa en la estadificación inicial del cáncer de cabeza y cuello, permitiendo modificar el estadio en el 42 % y el tratamiento en el 15 % de los casos. Su alto valor predictivo negativo (>99 %) contribuye a evitar procedimientos invasivos durante el seguimiento. La detección de tumores sincrónicos refuerza la necesidad de confirmación histológica sistemática.
