Estado psicoafectivo en pacientes sometidos a laringectomía total: estudio observacional

Resumen

Introducción y objetivo: La laringectomía total continúa siendo un tratamiento fundamental en el cáncer de laringe avanzado, aunque se asocia a importantes secuelas funcionales y posibles alteraciones psicoafectivas. La valoración sistemática del estado emocional no siempre forma parte del seguimiento clínico habitual. El objetivo de este estudio fue describir el estado psicoafectivo de pacientes sometidos a laringectomía total y analizar su relación con variables clínicas y terapéuticas.

Método: Se realizó un estudio observacional, transversal y descriptivo con componente analítico en pacientes sometidos a laringectomía total entre 2015 y 2024. La evaluación se llevó a cabo en un único momento del seguimiento postoperatorio, al menos un año después de la cirugía. Los síntomas de ansiedad y depresión se evaluaron mediante la escala Hospital Anxiety and Depression Scale (HADS). Se analizaron variables clínicas como el uso de psicofármacos, la derivación a servicios de salud mental, el uso de prótesis fonatoria y la administración de tratamiento oncológico adyuvante.

Resultados: Se incluyeron 33 pacientes. El 18,2 % presentó ansiedad probable y el 24,2 % depresión probable. El 30,3 % recibió tratamiento psicofarmacológico habitual y el 33,3 % fue derivado a servicios de salud mental. No se observaron asociaciones estadísticamente significativas entre la sintomatología psicoafectiva y el uso de prótesis fonatoria ni con el tratamiento oncológico adyuvante.

Discusión: La presencia de sintomatología psicoafectiva en esta cohorte pone de manifiesto la variabilidad en la adaptación emocional de los pacientes tras una laringectomía total, más allá del periodo postoperatorio inmediato.

Conclusiones: Una proporción relevante de pacientes laringectomizados presenta síntomas clínicamente significativos de ansiedad y depresión. La evaluación estructurada del estado psicoafectivo puede facilitar la identificación de pacientes que podrían beneficiarse de un abordaje multidisciplinar durante el seguimiento clínico.
