The Right to Health and Inequality: Primary Health Care Responses to the COVID-19 Pandemic in Northeastern Brazil
Abstract
In the context of the COVID-19 syndemic, characterized by the high burden of communicable diseases, chronic diseases, and various forms of violence, an interdisciplinary group composed of researchers, science communicators, and human rights advocates was established in Pernambuco, Brazil, in March 2020. The group was organized to develop initiatives aimed at promoting health care, formulating health care strategies, strengthening management, and defending human dignity. The initiative was based on institutional support for the development of pandemic response plans addressing clinical, epidemiological, social, subjective, political, and symbolic dimensions. This study aims to analyze the responses of Primary Health Care to the pandemic, considering the clinical, social, subjective, and health surveillance dimensions. The research was conducted in the Metropolitan Region of Recife, an area characterized by high levels of social vulnerability, using a mixed-methods approach that combined questionnaires with in-depth interviews involving health workers and users of Brazil's Unified Health System (Sistema Único de Saúde – SUS). The findings revealed significant challenges in ensuring the right to health, particularly regarding clinical care, health surveillance, social conditions, and subjective dimensions. The study concludes that strengthening institutional support and promoting changes in health care practices are essential both for responding to the COVID-19 pandemic and for preparing for future public health emergencies.
