Nursing Assessment of Lactation-Related Breast Pain. A Scoping Review
Abstract
Introduction: Breastfeeding is a valuable public health prevention tool. However, difficulties arise that lead to its discontinuation. Given the lack of consensus regarding the origin and treatment of breast pain during lactation, it is pertinent to address the topic from a nursing perspective.
Objectives: To review the evidence related to lactational breast pain in order to identify its causes, characterize it, and determine how to carry out the nursing assessment.
Materials and Methods: A literature search was conducted in PubMed, Web of Science, CINAHL, and the Nursing and Allied Health Database. The research question was formulated following the PIO model, and the search terms, inclusion and exclusion criteria, and relevant limits were determined.
Results: 144 publications were retrieved and 14 were selected. Breast pain is very common in the first 15 days postpartum. Its prevalence is difficult to determine because it is studied in conjunction with other pathologies. Its causes are diverse, and the most commonly used assessment tools are medical history, observation of breastfeeding, and numerical rating scales. Physiotherapy and interdisciplinary collaboration appear to be beneficial for its management.
Conclusions: Breast pain is common and multifactorial. Its prevalence varies, and there is no consensus regarding the best assessment tool or approach. The role of the nurse and their continuing education are highlighted. It is imperative to continue researching the origin, prevalence, assessment, and management of lactational breast pain.
