Beneficios autoinformados del uso de audífonos en la pérdida auditiva relacionada con la edad: estudio comparativo con pacientes con enfermedad de Ménière

Resumen

Introducción: La presbiacusia, o pérdida auditiva relacionada con la edad, es un tipo de pérdida auditiva neurosensorial progresiva e irreversible que afecta principalmente a las frecuencias altas del espectro auditivo. Se trata de una afección multifactorial común en los adultos mayores, que puede tener un impacto significativo en la calidad de vida, contribuyendo al aislamiento social, la depresión e incluso la demencia. La importancia de esto radica en el impacto que tiene en la salud mental, física y social de los pacientes, lo que puede ser crucial para mitigar los efectos adversos. El tratamiento de esta patología se centra principalmente en la rehabilitación auditiva y el uso de dispositivos, lo que mejora significativamente la comunicación y el nivel de vida. Los audífonos son la primera línea de intervención para la presbiacusia. Para aquellos pacientes que padecen una pérdida auditiva grave en la que los audífonos no son eficaces, se recomendaría el uso de implantes cocleares. Material y métodos: En este estudio se seleccionaron a dieciocho pacientes con presbiacusia que utilizaban audífonos bilaterales y que acudieron al servicio de Otorrinolaringología del Complejo Hospitalario Universitario de Salamanca entre septiembre de 2023 y agosto de 2024. Se recopilaron variables demográficas y se realizó a los pacientes una audiometría de campo libre con audífonos, una audiometría sin audífonos, una logoaudiometría con audífonos y otra sin audífonos. Además, se realizaron dos cuestionarios a estos pacientes: THI (Inventario de Incapacidad por Tinnitus) e IOI-HA (Inventario Internacional de Resultados para Audífonos). Se seleccionaron otros dieciocho pacientes con enfermedad de Ménière que utilizaban audífonos como segundo grupo, ya que estos pacientes tienen un tipo diferente de pérdida auditiva con características diferentes. Resultados: Se seleccionaron dos grupos de 18 pacientes (presbiacusia y enfermedad de Ménière). Aunque los umbrales audiométricos fueron en general mejores en el grupo de Ménière, los pacientes con presbiacusia mostraron un reconocimiento del habla superior con audífonos La SRT media fue menor en el grupo con presbiacusia (50,55 ± 9,98 dB) en comparación con el grupo con síndrome de Ménière (57,22 ± 9,40 dB), con una diferencia media de −6,67 dB (IC del 95 %: −13,23 a −0,11; p = 0,024). Discusión: En nuestra muestra, los pacientes con presbiacusia mostraron peores resultados en la audiometría con y sin audífonos en comparación con el segundo grupo, pacientes con enfermedad de Ménière. Sin embargo, en logoaudiometría se observa una tendencia opuesta. A pesar de tener una peor audición, los pacientes con presbiacusia muestran un rendimiento y una comprensión significativamente mejores que el segundo grupo. En las pruebas realizadas, la puntuación en el rendimiento de los audífonos es significativamente mejor en los pacientes con presbiacusia que en los pacientes con enfermedad de Ménière. Conclusión: Con este estudio, pretendemos demostrar la importancia de una adaptación adecuada de los audífonos.
