In silico characterization of clozapine pharmacokinetics in different patient populations.

Abstract

Clozapine is an atypical antipsychotic drug with a narrow therapeutic margin (0.35 mg/L – 0.60 mg/L) and with a risk of rare but high-risk side effects, which is why it is necessary its monitoration. With the advance of the design of applicatoins and computer programs, pharmacokinetic simulations of different molecules can be performed to estimate their behavior in the human body. The aim of this study is to perform varios simulations in silico, modifying clearance parameters according to age, sex or smoking habit, comparing the results obtained with population-based trials. SymCYP® v16.1 is a software that has libraries of physicochemical data on varios substrates and population data that, together with the pharmacokinetic data obtained from the literatura review, allow the simulation of pharmacokinetic curves and pharmacokinetic parameters of clozapine in a given population. The results obtained show significant differences in the serum concentrations of clozapine according to sex and age, with dose adjustments being necessary to maintain concentrations within safe levels. In conclusión, this work offers a new visión to the development of new molecules thanks to the use of massive in silico simulations.
