Análisis de incidencias en la comunicación interprofesional en un Centro Sociosanitario de Castilla y León en relación con el servicio que presta la Farmacia Comunitaria

Abstract

Pharmacy plays a crucial role in the safety of institutionalized patients, dispensing medicines and medical devices prescribed by the physician at the Social Health Center and by the Primary Care Physician. The objective was to identify incidents in interprofessional communication that could affect Pharmaceutical Care in a Social Health Center in Castilla y León. The community pharmacy recorded the incidents during the period from February to April 2024, evaluating 173 monthly treatments, with an average of 8±3 medications per treatment. 567 incidents were documented, distributed in treatment starting (119), withdrawals (124), change of regimen (119) and lack of renewal (205). 67% of the incidents were related to treatments containing between 7 and 14 medications, with those of the Nervous System being the ones that generated the most incidents (43%). It is concluded that improving interprofessional communication requires reducing the number of medications per treatment, especially those for the Nervous System, as well as the implementation of communication systems coordinated with the Pharmacy as responsible for dispensing. In fact, the incidents described indicate that the Professional Pharmaceutical Assistance Conciliation Service should be applied to institutionalized patients.
