The initial ultrasound diagnosis was of a lipoma or sebaceous cyst, with pathology being the key for the final diagnosis. Results: to highlight the diagnostic difficulty of epidermal lesions in the cervico-facial area and to study pilomatrixoma as a differential diagnosis. Discussion: pilomatrixoma is defined as one of the most frequent tumors in the cervicofacial area in pediatric age. Surgical removal is the treatment of choice and the recurrence rate is minimal. Conclusions: it is necessary to know pilomatrixoma as a frequent pediatric tumor in the head and neck area.
